You may create an account and have full access to this site or remain an unregistered user. Both will let you select services after answering a few questions. Based on your answers we will help you match to the best provider in our system.
As a registered user, you will have an account created and will be able to remain in contact with the provider you chose. You will be able to view upcoming appointments, take assessment, surveys and message the provider. You will be able to see the status of your pending request for services and you can also find more services for other needs you have. You will also be able to leave reviews based on your experience with the services you received.
As unregistered user, an account will not be created but after providing your contact information you will be notified by the provider once accepted.
Assessment Questions
Parents of youth from birth to age 17: Please answer the assessment questions about your child needing help and your family to help us understand your needs. Your answers will help us connect you to recommendations for services/programs to help you. The questions are based on an assessment created by the American Psychiatric Association, and it is called the Diagnostic and Statistical Manual V Level 1 Cross-Cutting Symptom Measure. Other questions are added to find out more about your child and family. After completing the questions, you will see a screen listing all the services that might help you. You may choose the ones you want to try. If you would like more options than your list, please click on Refine the search. The questions you have answered (assessment) will be sent to those agencies you select. Once you have selected the agency (or agencies) with whom you would like to work with, a representative from that agency will contact you to discuss next steps and schedule your first appointment. This may take a couple of days.
If you feel like you need help for yourself or your child right away (if someone is talking about killing themselves or others for example), please call 211 to talk to a counselor or you can text #741741. There is always someone to talk to even in the middle of the night. If you or your child is currently in a medical crisis, please call 911.
